Consistently disable small-code checks.

* gas/config/tc-m68k.c (FLAG_SMALL_CODE): New define.
  (m68k_ip): Replace checking of flag_small_code with FLAG_SMALL_CODE.
  (md_convert_frag_1): Likewise.
  (md_estimate_size_before_relax): Likewise.
